The following is a summary of the benefits offered by the Cultural Insurance Services International, a leader in insurance for studying and travelling abroad. Please contact us with any additional questions or to request the additional insurance benefits described below.
Basic
Plan
Medical expenses
If, as the result
of a covered Injury or Sickness which occurs while insured under this plan,
you require treatment by a Physician or surgeon, confinement in an infirmary
or Hospital, services of a registered nurse, X-rays, ambulance service or any
covered services, supplies or medicines prescribed by a Physician, the plan
will pay usual, customary and reasonable expenses in excess of a deductible
of $100 per incident incurred within 52 weeks of date of Accident or commencement
of Sickness up to a maximum of $50,000 for each Accident or each Sickness. You
must remain continuously insured for the duration of the treatment.
Hospital room and
board maximum is the average semi-private rate.
Chiropractic and
therapeutic services shall be limited to a total of $50 per visit, with a maximum
10 visits per Injury or illness. The overall maximum coverage per Injury or
illness for chiropractic care is $500, which includes X-ray and evaluation charges.
Benefits for nervous
or mental disorders shall be limited to (a) up to $500 for outpatient treatment
or (b) up to $2,500 on an inpatient basis.
Accidental death
and dismemberment
If injuries result
in death or dismemberment within one year after the date of Accident, the plan
provides these benefits for loss of:

Only one benefit,
the largest to which you are entitled, is payable for losses from the same Accident.
Please note: The death benefit will be paid to your estate unless you provide
the name and relationship of your beneficiary at time of enrollment.
Travel
Assistance Service
A 24-hour travel
assistance service is at your disposal staffed by multilingual coordinators
trained to assist you should an emergency arise while you are overseas.
Direct hospital
payments
Many Hospitals
require Americans to make large deposits before receiving any medical care and
require full cash settlement at time of discharge. The travel assistance service
will provide guarantees to Hospitals enabling immediate admission to Hospital
care, if necessary.
Overseas medical
assistance
Should you require
emergency local care, the coordinator will refer you to an appropriate Physician
or medical facility.
Medical consulting
and monitoring
If you are hospitalized,
the coordinator will monitor your stay to ensure appropriate care. Contact is
maintained with the patient and local Physician. If necessary, your family or
personal Physician will be advised.
Legal assistance
A coordinator can
help you find a local legal advisor overseas. If posting bail or immediate payment
of legal fees is required, arrangements for a monetary advance can be made through
many major credit cards or from your family.
Emergency cash
transfer
If your cash or
checks are lost or stolen, or if cash is needed to cover an unexpected expense,
the coordinator can help arrange for emergency funds to be made available through
many major credit card accounts or from your family.
The following
services are payable to a combined limit of $50,000:
Medical transportation
When medically
necessary and coordinated by the travel assistance service, evacuation costs
to the nearest appropriate medical facility will be paid.
Emergency medical
evacuation
The Assistance
Provider will pay for covered expenses incurred if an accidental Injury or Sickness
commencing during the course of the plan coverage period results in your necessary
emergency evacuation. An emergency evacuation must be ordered by a Physician
who certifies that the severity of your accidental Injury or Sickness warrants
the emergency evacuation.
Repatriation
The Assistance
Provider will pay the reasonable covered expenses incurred to return your body
to the U.S. if you die during the plan coverage period. Covered expenses include,
but are not limited to, expenses for embalming, cremation, minimally necessary
casket for transport and transportation.
Optional
Extra Protection Plan
In addition to
the basic plan, you may purchase an insurance upgrade prior to your departure.
The cost of the Extra Protection Plan is $100 per semester. A semester is any
period from one to six months. The Extra Protection Plan contains these additional
benefits:
Medical expenses
Your maximum medical
benefit will be increased to $100,000 (from $50,000) for usual, customary and
reasonable medical expenses. The per incident deductible will be reduced from
$100 to $0.
Baggage and personal
effects
Extra protection
covers loss or theft of baggage or personal effects up to $1,000. A $50 deductible
per claim will apply. The limit per item is $100, except cameras, $250.
Emergency family
reunion
If you are hospitalized
for at least seven consecutive days due to a covered Injury or Sickness, the
Company will pay up to $1,500 for round trip air fare to the host country on
a regularly scheduled commercial flight (tourist class) for a parent or legal
guardian and hotel and meals up to $100 per day. Any expense in respect to an
emergency family reunion requires prior approval of the Company.
Tuition refund
The Company will
pay 50% of any unreimbursed tuition expense up to $1,000 if you suffer a covered
Injury or Sickness which prevents you from taking final examinations or make-up
examinations. The condition must last at least four consecutive weeks and be
certified by a Physician.
